Side Effects for Surprise: On India’s weight loss drug revolution, Dr. Embrish Mithil
In this episode of ‘booked’, the top endocrinologist Dr. Embrish Mithal unpacked the world of new age weight-management drugs, their side effects, their benefits and their new book The Wet Los revolution.

India’s struggle with rising obesity and diabetes is well documented. But a revolution is now going on, one is how we manage both situations: GLP-1-based weight loss drugs.
India’s leading endocrinologist and his recently published book The Weight Los Revolution, Dr.. Embrish Mithal explains why these drugs are meant to live here, wrong information with facts.
Beyond Metformin: A new era in taking care of diabetes
Over the years, the standard has been the first-rich treatment for metformin type 2 diabetes. It is safe, inexpensive and effective. But new drugs like GLP-1 receptor agonist and SGLT-2 inhibitors are rapidly changing that equation.
“With these drugs, you are moving far beyond the metformin because there is a clear evidence that these drugs produce significant weight loss that they protect your heart, they protect your kidneys, they also protect your liver.
Most people with type 2 diabetes in India are about 80 to 90%, weighing over. This makes them an ideal candidate for GLP-1 drugs, which helps in weight loss when managing blood sugar levels.
“Bulk, type of type 2 diabetic patients are overweight, need to lose weight, and lose weight will also help reverse them or at least better treatment of their diabetes,” Dr. Mithal says.
Not all fat are the same
Traditionally, body mass index (BMI) has been used to assess weight -related health risks. But Dr. Mithhal explains that for Indians, BMI is misleading.
“If you only use BMI, you will be classified as a lot of Indians as healthy when they actually have too much central fat or intestine fat,” he said.

This dangerous “abdominal fat”, also known as intestinal fat, triggers inflammation throughout the body. Dr. Mithhal described this distinctive South Asian pattern as toffee – thin, fat inside.
GLP-1 is not a magic pill
GLP-1 drugs in India are now sold as Mounjaro and Wegovy, and semaglutide tablets are not quick-fix solutions. Dr. Mithhal stress should be part of a structured program that includes diet and exercise.
“If you are using these drugs for weight loss, do not depend on the drug alone,” they say.
To do this, he recommends going on a high protein intake, adds more fiber to the diet, has small food throughout the day and walking after meals.
Muscle loss risk
Rapid weight loss, especially without strength training, can cause muscle loss. “If you lose 10 kg, you lose 2-3 kg of muscles,” Dr. Mithal has warned.
“If you use the medicine as a weight loss medicine and are not doing anything else, you will also lose muscles.”

He recommends strength training and adequate protein to maintain muscles and long -term health benefits.
Ozapic face and other side effects
The popular word “ozmpic face” refers to slapping the skin of the face due to sudden weight loss, not unique to these drugs.
“Before these drugs enter the market, any person we keep on a very strict diet or someone who had bariatric surgery will have similar things.”
Most side effects include gastrointestinals such as nausea, vomiting and rare concerns include marrow thyroid cancer, although the link remains unwanted.
Happy side effects of new age drugs
GLP-1 drugs have also known evidence of benefiting patients more than weight and diabetes.
“The data of these drugs is Alzheimer’s Dissease, Better Heart Health, Parkinson’s disease,” Dr. Mithal says. “Sleep apnea goes down, fatty liver decreases, and even the progression of kidney disease decreases.”

They can also improve fertility in women with PCOS. However, drugs can reduce the effectiveness of birth control pills, causing unexpected pregnancies, known as ozmpic infants.
Not for all and not forever
Dr. Mithhal says that these drugs should not be used as public health tools. Prevention through lifestyle is still the best way to fight obesity. Dr. “You cannot erase obesity using these drugs, which you have to stop obesity by using these medicines,” says.
In addition, the long -term use of these drugs depends on age, health goals and the cause of use.
“If you are a 30 -year -old baby, you should definitely not use a drug life. I will take a deep breath after two years and see what I do now?” He says.
Currently, most GLP-1 drugs are injected and expensive, limiting their access to India. However, new yoga and more research are in the way.
“Remember, the GLP-1 area is simply exploding. There will be many new drugs with low and low side effects,” Dr. Dr. Mithal says.
